To apply for the tournament, you may either fill out the following form and submit it online, or print out a hard copy using the link below. Please fill out the form in its entirety and click submit once. We look forward to seeing you there! Credit card payments can be made at the door on day of tournament.

Please remember to submit a payment by mail. Your application will not be considered complete until your payment has been received and processed.
